First Appointment

We wa​nt to make your visit with us both easy and pleasant. To help serve you better, we recommend reviewing the following:


What do I need to bring with me?

  1. All relevant medical records, including imaging studies or reports.

  2. Your insurance identification card so that we may copy the front and back of the card for accurate insurance information.

  3. Your driver’s license so that we may copy the card for accurate demographic and patient specific data.

  4. If you have a health plan that requires its own insurance claim form, please provide us with a signed and completed claim form.

  5. Your referral or authorization for services when applicable.

  6. Filled out New Patient Forms (optional)


Do I need to wear anything in particular?

It’s important that you’re comfortable. Your session may include stretches and moderate exercise, so dress as if you’re going to a yoga class or the gym. If coming in work attire realize it could be difficult to get you in positions we may need to test or have you move in.


What’s your cancellation policy?

We have a 24-hour cancellation policy. There is a $25 fee if you do not let us know a day in advance that you will not be coming to your physical therapy or other subspecialty visit. There is a $100 fee if you do not cancel your doctor’s appointment 24 hours before you are scheduled to see one of our staff doctors.
